Nottingham County Council operate a hospital discharge scheme in Mansfield and Ashfield (audio link)
The Hospital Discharge Scheme provides support to patients and their carers when discharged from hospital. It is run by paid staff and volunteers who will visit you in your home and provide a range of help.It is available in the Ashfield and Mansfield districts, and some of the surrounding areas.
You do not need a social worker to qualify for this support. It is a free and confidential service
